Cauliflower ear: what happens, and the protection that is banned

It is the most recognisable mark of someone who has grappled for years. Behind it is a specific mechanism, and a contradiction: the only protection that works cannot be used in competition.

Careful This page explains a phenomenon, it does not teach you to manage it. A swollen ear after training is something to have looked at by a doctor, not something to sort out with what you read on a forum or hear at the gym.

Cauliflower ear is not a bruise that never goes away. It is the result of a process: trauma causes blood to collect between the cartilage and the lining that feeds it. If that lining — the perichondrium — stays separated, the cartilage loses its blood supply, and fibrous tissue forms in place of the original tissue. The deformity you see is that.

How common it is

Far more common than it looks. In a study of national-level martial artists, 84% of the men had cauliflower ear, and 96% had suffered ear trauma. Among high-level German judoka the figure drops but stays high: 55.5%.

The protection works

The literature on wrestlers reports up to 50% fewer ear haematomas with the use of ear guards. It is one of the few clear numbers in this whole section.

And in competition it is banned

In the IBJJF rulebook, as at the ADCC, ear guards are banned in competition, along with headgear, glasses, groin cups and any rigid material. The reason for the ban is not medical: it is that padding changes grips and the bulk of the body.

Note In training they are allowed, and training is where most of the trauma accumulates — since training is where almost all the time is spent. So anyone who wants to avoid it has a tool available exactly where it matters most.

Careful ⚠️ How many people actually wear them, nobody knows: there are no surveys on ear guard use in jiu-jitsu. That they are uncommon is a gym observation, not a measured figure.

One point that matters

Cauliflower ear gets talked about as a badge, and in part that is what it has become. But it is worth knowing that it is the result of tissue that does not repair itself: the shape it takes is permanent. Choosing to live with it is a legitimate choice; it is useful for it to be an informed choice, rather than the outcome of never having heard that a protection existed.
